The Product Failure-to-Learning Compiler

Build a product learning system that converts defects, outages, customer complaints, failed experiments, and support escalations into reusable engineering and product knowledge.

Allow teams to import incident reports, bug trackers, customer feedback, deployment notes, and postmortems. Extract the observed failure, affected users, triggering conditions, contributing factors, recovery actions, and evidence that supports each conclusion.

Connect recurring failures across releases and components. Identify repeated assumptions, missing tests, unclear ownership, inadequate monitoring, and corrective actions that were proposed but never completed.

For each incident, generate a structured learning record containing verified facts, unresolved questions, likely contributing factors, preventive controls, and regression tests. Link recommendations to original records rather than inventing a root cause.

Include a search interface for finding similar historical failures during planning and code review. Track whether preventive actions actually reduce recurrence, and distinguish correlation from demonstrated improvement.

Support privacy controls, versioned records, and exportable engineering lessons that teams can reuse across future projects.
